Student Profile Forms

This is the form counselors use to write student letters of recommendation. The first version is for electronic submission, and the second version allows students to download the document, hand write their answers and then return the form to the appropriate counselor upon completion.

Items Needed to Register

 

This is a list of items needed to complete your student registration or withdrawal process. Bring copies of all these required documents to expedite the process:

Enrollment

In District Student Registration Requirements

(Students that currently attend a DeKalb County School District School)

  • Proof of residency (please bring a copy for the schools record)
  • Immunization Form 3231 (please bring a copy for the schools record)
  • Vision/Hearing/Dental Form 3300 (please bring a copy for the schools record)
  • Parent photo identification (please bring a copy for the schools record)
  • A copy of the student's social security card (please bring a copy for the schools record)
  • A copy of the student's birth certificate (please bring a copy for the schools record)

Out of District Student Registration Requirements

(Students that attend a private or any other school outside of DeKalb County District or home school)

  • Proof of residency (please bring a copy for the schools record)
  • Immunization Form 3231 (please bring a copy for the schools record)
  • Vision/Hearing/Dental Form 3300 (please bring a copy for the schools record)
  • Parent photo identification (please bring a copy for the schools record)
  • A copy of the student's social security card (please bring a copy for the schools record)
  • A copy of the student's birth certificate (please bring a copy for the schools record)
  • An official discipline record sealed (please bring a copy for the schools record)
  • An official attendance record sealed (please bring a copy for the schools record)
  • An official transcript record sealed (please bring a copy for the school's record)

Withdrawal

  • Parents must have valid identification. (Only the parent/legal guardian that registered the student will be allowed to withdraw this student)
  • Students must be present to clear locker, return all textbooks to the registrar, and clear any media center or lunch room holds.
  • Only cash or money orders are accepted as payment for fines or lost materials.

To update a student's address, you will need to submit proof of residency and your valid identification.

Proof of Residency includes:

  • Electric Bill
  • Water Bill
  • Gas Bill
  • Signed Mortgage Statement or Rental Agreement
