The 2023-2024 Georgia Student Health Survey, Georgia Parent Survey, and the Georgia School Personnel Survey is now open. The Georgia Student Health Survey (GSHS) is an anonymous, statewide survey instrument created by the Georgia Department of Education (GaDOE) and administered online each year to students in grades 3-12. These surveys will contribute to the 2024-2025 School Climate Score. 

Per the Protection of Pupil Rights Amendment (PPRA), parents/guardians may review the student survey items and have the option to “opt out" of survey participation by returning a signed consent form (see link below) to the student's respective school. See links to the survey items and form below:
